چکیده

CAP value among the 3 groups. MANOVA multivariate analysis evaluated differences in overall CAPs and the effects of age, gender, Mueller grade and stings number. A P , 0.05 was considered statistically significant. Data were analysed by ‘SPSS’ 13.0 (SPSS Inc., Chicago, IL). We called NS patients to check if they were stung after VIT discontinuation. Results: We selected 84 NS, 72 SP3 and 76 SP5 patients. Specific YJ-IgE levels decreased during VIT, as CAPs are statistically different at time 0, 3 and 5 (P , 0.001). Considering CAP levels at the first control (CAP3), NS patients presented lower values than SP3 patients (P 1⁄4 0.002); no significant difference was found between NS and SP5 patients. At the last control (CAP5), CAP values of NS and SP5 patients were different (P 1⁄4 0.002) as well as between SP3 and SP5 patients (P 1⁄4 0.014). No significant difference was found between NS and SP3 patients. By MANOVA, IgE decrease was inversely correlated with Mueller grade (P 1⁄4 0.012) and age (P 1⁄4 0.002). We recalled all NS patients by phone, 7/84 (8.3%) patients related one well tolerated sting, as they did not develop any allergic reaction. Conclusions: In everyday practice if a patient never stung during VIT fulfils the temporal criterion, but specific IgE are not negative, a decrease of IgE levels ranging from 57 to 70% in respect to baseline might be a satisfactory parameter for stopping VIT. As a further confirmation of our conclusions, even if not statistically significant, all not stung patients were clinically protected after VIT discontinuation.

Age-dependent sting recurrence and outcome in immunotherapy-treated children with anaphylaxis to Hymenoptera venom.

BACKGROUND Data on outcome of insect venom immunotherapy in children are rare. OBJECTIVE We investigated the rate of sting recurrence and outcome of Hymenoptera venom anaphylaxis in children of different age groups treated with immunotherapy. METHODS Data from children consecutively referred for anaphylaxis to Hymenoptera venom were collected using a standardized questionnaire. RESULTS Du...

Dose-dependence of protection from systemic reactions to venom immunotherapy by omalizumab

BACKGROUND Systemic reactions (SR) to venom immunotherapy (VIT) are rare but may occur, with a rate significantly higher for honeybee than for vespid VIT. In patients with repeated SRs to VIT it is difficult to reach the maintenance dose of venom and pre-treatment with omalizumab is indicated, as shown by some studies reporting its preventative capacity, when antihistamines and corticosteroids ...

Bee venom hypersensitivity and its management: patients perception of venom desensitisation.

The objectives of the study were to review bee venom immunotherapy from the patient's perspective: in particular its benefits and its problems, and to investigate any genetic tendency for bee venom hypersensitivity. A self administered, 9 item questionnaire was sent to 219 patients who had undergone either inpatient or outpatient bee venom immunotherapy at Flinders Medical Center. The clinic re...

Severe anaphylaxis to bee venom immunotherapy: efficacy of pretreatment and concurrent treatment with omalizumab.

Immunotherapy is an established mode of treatment for Hymenoptera venom anaphylaxis, although adverse reactions may occur. We report the case of a 33-year-old woman, the wife of a beekeeper, who experienced a systemic allergic reaction following a bee sting. Initial specific immunotherapy had to be stopped due to anaphylaxis (multiple immediate cardiovascular reactions). We looked for an altern...

In vitro Biphasic Effect of Honey Bee Venom on Basophils from Screened Healthy Blood Donors

Apis mellifera L. bee venom is the most studied hymenoptera allergen, but many aspects of its action on human basophils remain unclear. Allergologists seek evidence of the effectiveness of bee venom immunotherapy as this approach is the chosen treatment for systemic allergic reactions. The effect of bee venom on human basophils in vitro has not been studied in detail for many reasons, including...
